In a double blind, randomized controlled trial, 170 women, mean age 36, took 20mg per day of a 6-12:1 extract ratio (brand name Ze440). After taking for 3 menstrual cycles, there was a decrease in overall PMS symptoms as well as irritability, mood alteration, anger, and feelings of breast fullness compared to placebo. [1]
In a double blind, randomized controlled trial, 162 women with PMS, ages 18-45, took 8 mg, 20mg, or 30mg of a chasteberry extract (brand name Ze440 6-12:1 extract ratio) for 3 menstrual cycles. Compared to placebo, there was a significant decrease in total PMS symptom score and individual scores of irritability, mood alteration, anger, feelings of breast fullness for both doses(p<0.001)[2]
A double blind, randomized controlled trial of 57 women took 20mg or 30mg of a chasteberry extract (brand name Ze440 6-12:1 extract ratio) per day or a placebo. After 2 menstrual cycles, the chasteberry groups had improvements in all endpoints compared to baseline, including mood swings and general emotional feelings. [3]
A double blind, randomized controlled trial of 407 women took 1 chasteberry extract (brand name Agnolyt) capsule per day, 1 placebo twice daily, or 1 capsule of pyridoxine-HCl (vitamin B6) (100mg) twice daily on days 16-35 of their menstrual cycle. After 3 menstrual cycles, the Agnolyt and B6 groups had reduction in scores of PMS symptoms. 36.1% of the Agnolyt group and 21% of the B6 group participants had no PMS complaints at the end of the study. In comparison to the B6 group, Agnolyt was superior in alleviating PMS complaints, including breast tenderness, bloating, inner tension, and mood problems.[4]
A systematic review and meta-analysis of 17 randomized controlled trials of Vitex agnus castus for the treatment of premenstrual syndrome explored pooled treatment effects were large but recommended additional research.[5]

Vitex agnus castus “seems to affect the hypothalamic–pituitary axis and consequently reduce the release of follicle-stimulating hormone (FSH) and prolactin, and modulate the release of luteinizing hormone (LH) from the pituitary gland. This effect can increase the blood progesterone and modulate the prolactin.”[6]
Flavonoid compounds such as extracted apigenin from V. agnus-castus were identified as the most active phytoestrogens for estrogen receptors, especially beta-estrogen receptors, and the effectiveness of flavonoids on estrogen receptors was observed experimentally.[7][8]
